1. What is Epiphany Health Direct Primary Care?
Epiphany Health Direct Primary Care is a membership-based primary care practice that offers comprehensive primary care services. It provides a direct relationship between patients and their primary care physicians without involving insurance companies or third-party payers.

2. How does Epiphany Health Direct Primary Care work?
Patients pay a monthly or annual membership fee, which covers a wide range of primary care services provided by their designated physician. This includes routine check-ups, preventive care, chronic disease management, acute care, and access to certain additional services. It includes in-office testing, procedures, and telehealth services.

3. What are the benefits of joining Epiphany Health Direct Primary Care?
Some benefits include increased access to your primary care physician, extended appointment times, shorter in-office wait times, same-day or next-day urgent appointments with YOUR doctor, enhanced preventive care, reduced out-of-pocket costs, and improved continuity of care.

4. Do you accept my insurance, and do I still need health insurance if I join?
Epiphany Health Direct Primary Care is not a substitute for health insurance. It focuses on providing primary care services, but it does not include specialized care, hospitalizations, or emergency services. Patients are encouraged to have health insurance to cover those needs.  While we DO NOT bill any insurance companies for our services, most of our patients still carry insurance. For those without insurance, Epiphany Health does offer a much-needed affordable solution for routine care.

5. Can I use my health insurance alongside Epiphany Health Direct Primary Care?
Yes, you can still use your health insurance for services outside the scope of Epiphany Health Direct Primary Care, such as specialist visits, hospitalizations, and emergency care. Your primary care physician can coordinate and collaborate with other healthcare providers involved in your care.

(Note: HMO insurance requires referrals to go through an in-network primary care physician)

6. What is the cost of membership for Epiphany Health Direct Primary Care?
Membership fees vary depending on age. Check here for specific pricing and membership options.

7. Are there any additional fees or hidden costs?
Generally, the membership fee covers most primary care services. However, there may be additional charges for certain procedures, laboratory tests, vaccines, medications, or other services not included in the membership. 8. What if I need to see a specialist or require further diagnostic tests?
Your primary care physician at Epiphany Health Direct Primary Care will coordinate your care with specialists and arrange any necessary diagnostic tests. We will work closely with you to ensure appropriate referrals and coordinate your overall healthcare needs. We work with our ancillary service partners to help you find the best available price, often cheaper than an insurance copay!  

9. Can I join Epiphany Health Direct Primary Care if I have pre-existing conditions?
Yes, Epiphany Health Direct Primary Care welcomes patients with pre-existing conditions. Our primary care physicians are experienced in managing chronic diseases and can provide personalized care plans to help you manage your condition effectively.

10. How do I become a member of Epiphany Health Direct Primary Care?
To become a member, you can reach out to Epiphany Health directly, either through our website or by calling 941-423-9936. Our new patient intake associate will provide you with the necessary information and guide you through the enrollment process.

11. What are your hours of operation?

Our office hours are Monday - Thursday 8:30 AM to 4:30 PM and Friday 8:30 AM to 3:30 PM.  We close for lunch daily from 12PM-1PM. 

12. What if I need help after-hours?

If it is a life threating emergency, call 911. For urgent concerns outside office hours, call 941-423-9936. Our phones are answered 24 hours per day through a live answering service. You can leave us a message, or If you need to speak to one of our physicians, ask the service to page our covering on-call physician. Please save non-urgent questions or prescription refill requests for office hours or contact the pharmacy directly to generate a digital refill request.

13. Can I email your office?

Yes! Our HIPAA secure email is info@epiphanyhealth.org. Active members can email non-urgent requests, questions or photos that will be reviewed by our clinical staff at least once daily on business days. Do not send any urgent email without first notifying our team.
