Lee S. Gross, M.D. |
Lee S. Gross, M.D. is the founder of our North Port practice. He is a 6-time recipient of the North Port Sun Readers' Choice award for Best Physician. He received his Bachelors degree from The Ohio State University. After 3 years of clinical research for the cardiology department of the Cleveland Clinic Foundation, he then earned his MD from Case Western Reserve University. After graduating from medical school he completed his residency in Family Medicine at University Hospitals of Cleveland, where he was elected chief resident. He has been Board Certified by the American Board of Family Medicine and has been in private practice in North Port since 2002.
After North Port was turned down for a hospital by the State in 2004, he was instrumental in the development of North Port’s 24 hour emergency care facility, now owned and operated by Sarasota Memorial Hospital, and continues to seek out ways to offer care to the under-served.
Dr. Gross is a national speaker for Direct Primary Care (DPC). He is the President of the Docs 4 Patient Care Foundation, a national non-profit organization dedicated to preserving the sanctity of the doctor and patient relationship. He has testified before the US Senate and been a consultant to The White House on healthcare reforms. He is the President of the Charlotte County Medical Society; a delegate to the Florida Medical Association; an appointee to the FMA Council on Medical Economics and Practice Innovation, and served on the Board of Trustees of Fawcett Memorial Hospital and the US Medical Association. Dr. Gross serves patients in our North Port location.

William L. Crouch, M.D. |
William L. Crouch, M.D. is originally from Bowling Green, KY. He earned his Bachelor of Arts degree from Vanderbilt University while majoring in Biology. Upon graduation he worked in research at the Stout Neuroscience Laboratory in Memphis, TN. He attended medical school at the University of Tennessee Health Science Center and received the NIH Medical Student Research Fellowship Award for clinical research on Type 2 Diabetes. He completed his Family Medicine Residency at Bayfront Medical Center in St. Petersburg, FL. He is board certified in Family Medicine.
After Residency, Dr. Crouch joined Pennyrile Family Physicians in Hopkinsville, KY. He was chief of the Department of Family Medicine at Jennie Stuart Medical Center. He also served on the Board of Directors for the Western KY Health Care System PPO. He partnered with Jenny Stuart Medical Center to launch the first afterhours urgent care in the community.
Dr. Crouch joined Dr. Lee Gross at Prime Health of North Port in 2007. Together they launched Epiphany Health Direct Primary Care; now regarded as a national leader in Direct Primary Care. Dr. Crouch and Dr. Gross have expanded their services to surrounding DeSoto County. Partnering with DeSoto Memorial Hospital, Epiphany Health now offers services to hospital employees, family members and the community. Since moving back to Florida, Dr. Crouch has also served as Medical Director for New College of Florida, as well as, a preceptor for Yale University Physician Assistant program. Dr. Crouch is currently accepting new patients.

Tiffany Hubman, DO|
Originally from West Virginia, Dr. Hubman joined Epiphany Health in 2021 to serve patients in our DeSoto County location. Dr. Hubman is a US Army veteran, having served in the combat military police company in Iraq during Operation Iraqi Freedom and Operation Enduring Freedom. She received her medical degree from the West Virginia School of Osteopathic Medicine. She completed her family medicine residency at Manatee Memorial Hospital and is certified by the American Osteopathic Board of Family Physicians. Dr. Hubman's character, genuine warmth and compassion make her a welcome addition to the Epiphany Health care team. Dr. Hubman serves patients in our DeSoto county location.

Candice Nicoll, ARNP|
Candice is a board certified Advanced Registered Nurse Practitioner and joined Epiphany Health in December of 2015. Born and raised in Canada, she completed her undergraduate education at Douglas College in Vancouver, British Columbia. Later she graduated from the University of South Florida with a Master of Science degree in Nursing. She is certified with the American Nurse Credentialing Center and the Florida Board of Nursing. Candice has a passion for patient-centered care. She has over a decade of primary care and women's wellness experience. She also has several years of experience in gastroenterology specialty care. Candice has expertise in the cure of hepatitis C with the latest advancements in treatment. Candice serves patients in our North Port location.

Anne Horner|
Anne is the Chief Operating Officer of Epiphany Health. Originally from Vermont, she has lived in Florida most of her life. She oversees the operations of all practice locations. She is also responsible for community marketing and practice development. She is the primary point of contact for new patients and business accounts. She has been with the practice since its initial founding in 2002, through its transition to DPC in 2010, and has served in nearly every capacity of the practice along the journey.
