Lee S. Gross, M.D. |
Lee S. Gross, M.D. is the founder of our North Port practice. He is a 5 time recipient of the North Port Sun Readers' Choice award for Best Physician. He received his Bachelors degree from The Ohio State University. After 3 years of clinical research for the cardiology department of the Cleveland Clinic Foundation, he then earned his MD from Case Western Reserve University. After graduating from medical school he completed his residency in Family Medicine at University Hospitals of Cleveland, where he was elected chief resident. He has been Board Certified by the American Board of Family Medicine and has been in private practice in North Port since 2002.
After North Port was turned down for a hospital by the State in 2004, he was instrumental in the development of North Port’s 24 hour emergency care facility, now owned and operated by Sarasota Memorial Hospital, and continues to seek out ways to offer care to the under-served.
Dr. Gross is a national speaker for Direct Primary Care (DPC). He is the President of the Docs 4 Patient Care Foundation, a national non-profit organization dedicated to preserving the sanctity of the doctor and patient relationship. He is the President of the Charlotte County Medical Society; a delegate to the Florida Medical Association; an appointee to the FMA Council on Medical Economics and Practice Innovation, and serves on the Board of Trustees of Fawcett Memorial Hospital. Dr. Gross serves patients in our North Port and DeSoto locations.
William L. Crouch, M.D. |
William L. Crouch, M.D. is Board Certified in Family Medicine. He received his Bachelors degree from Vanderbilt University in Nashville, Tennessee. Dr. Crouch received his Medical Doctorate degree from the University of Tennessee in Memphis. He completed his family medicine residency at Bayfront Medical Center in St. Petersburg, Florida.
After 5 years in private practice in his home state of Kentucky, he returned to the Sunshine State with his family. His strong clinical skills combined with his compassion and empathy explain why his patients continually rate him among the best family physicians in Florida. Dr. Crouch serves patients in our North Port and DeSoto locations.
Rachel Pandit, MD|
Dr. Pandit offers 20 years of experience in the field of medicine and healthcare administration. She has worked in a variety of roles and settings in the health care industry, both in the USA and abroad.
Dr. Pandit began her medical career in her home state of Missouri at the University of Missouri-Columbia where she received her medical degree in 1997. She went on to complete her residency in Family Medicine at the University of South Florida’s Bayfront Hospital in St. Petersburg in 2000. From 2000-2003 Dr. Pandit worked in Urgent Care for Bayfront Convenient Care. She then moved to California for 2 years and worked for Ventura County Health, caring for migrant and indigent patients, offering her expertise to this under-served population.
In 2007 she married a British Ophthalmologist, which took her to the Cayman Islands for 5 years, where she practiced medicine for the Health Services Authority. This was followed by a move to the United Kingdom with her family, where she operated a small health care campany, Moor Eye Care.
During this time she also studied nutrition and its effects on health. In addition to running the health care company she offered a nutritional medicine practice to people who wanted to incorporate a healthy lifestyle into their medical care.
After being away from the United States she and her family had a strong desire to return to Florida and start practicing traditional family medicine once again. She is happy to join Epiphany Health as it is a unique opportunity to offer direct primary care to patients who see the value of continuity.
She offers an eclectic background of experiences and brings a unique perspective to the treatment of common medical problems. Dr. Pandit is exclusively in our North Port location.
Candice Nicoll, ARNP|
Candice is a board certified Advanced Registered Nurse Practitioner and joined Epiphany Health in December of 2015. Born and raised in Canada, she completed her undergraduate education at Douglas College in Vancouver, British Columbia. Later she graduated from the University of South Florida with a Master of Science degree in Nursing. She is certified with the American Nurse Credentialing Center and the Florida Board of Nursing. Candice has a passion for patient-centered care. She has over a decade of primary care and women's wellness experience. She also has several years of experience in gastroenterology specialty care. Candice has expertise in the cure of hepatitis C with the latest advancements in treatment. Candice is exclusively in our North Port location.
Anne is the Chief Operating Officer of Epiphany Health. Originally from Vermont, she has lived in Florida most of her life. She oversees the operations of all practice locations. She is also responsible for community marketing and practice development. She is the primary point of contact for new patients and business accounts. She has been with the practice since its initial founding in 2002, through its transition to DPC in 2010, and has served in nearly every capacity of the practice along the journey.
Tiffany Hubman, DO|
COMING IN 2021!!
Originally from West Virginia, Dr. Hubman will be joining Epiphany Health to serve patients in our DeSoto County location. Dr. Hubman is a US Army veteran, having served in the combat military police company in Iraq during Operation Iraqi Freedom and Operation Enduring Freedom. She received her medical degree from the West Virginia School of Osteopathic Medicine. She is completing her family medicine residency at Manatee Memorial Hospital.
