Dr. Lee Gross

Lee S. Gross, M.D. is the founder of our North Port practice and a multiple-time recipient of the North Port Sun Readers' Choice award for Best Physician. Originally from Ohio, he obtained his MD from Case Western Reserve University and completed his residency in Family Medicine at University Hospitals of Cleveland, where he was elected chief resident. Dr. Gross has been Board Certified by the American Board of Family Medicine and has served the North Port community since opening the practice in 2002.

In 2004, after North Port was denied a hospital by the State, Dr. Gross played a vital role in establishing the 24-hour emergency care facility, now owned by Sarasota Memorial Hospital, to address the community's needs. He remains committed to expanding affordable access to care for the under-served throughout the region and across the nation through his leadership in innovation and expansion of healthcare market reforms.

Dr. Gross is a national speaker for Direct Primary Care (DPC), advocating for proven market-based healthcare solutions. He serves as President of the non-profit Docs 4 Patient Care Foundation. His expertise in health policy has led him to testify before the US Senate and consult with The White House on healthcare reforms.

 

Dr. Gross has held leadership positions in the Charlotte County Medical Society, the Florida Medical Association, and the FMA Council on Medical Economics and Practice Innovation. He has also served on the boards of an HCA Hospital, the US Medical Association, and the Chamber of Commerce.

With unwavering dedication, Dr. Gross continues to provide exceptional medical care to patients in our North Port location, embodying compassion, innovation, and advocacy.

Dr. William Crouch

William L. Crouch, M.D. | 

 

William L. Crouch, M.D., originally from Bowling Green, KY, is a board-certified Family Medicine physician. He began his academic journey at Vanderbilt University, where he earned a Bachelor of Arts degree in Biology. Following his undergraduate studies, Dr. Crouch gained valuable research experience at the Stout Neuroscience Laboratory in Memphis, TN.

Driven by his passion for patient care, Dr. Crouch pursued a medical degree at the University of Tennessee Health Science Center. During his time there, he was recognized with the NIH Medical Student Research Fellowship Award for his clinical research on Type 2 Diabetes. To further refine his skills, he completed his Family Medicine Residency at Bayfront Medical Center in St. Petersburg, FL.

After completing his residency, Dr. Crouch joined Pennyrile Family Physicians in Hopkinsville, KY, assuming the role of Chief of the Department of Family Medicine at Jennie Stuart Medical Center. Additionally, he served on the Board of Directors for the Western KY Health Care System PPO. Driven by his dedication to the community, he collaborated with Jenny Stuart Medical Center to establish the first after-hours urgent care facility.

In 2007, Dr. Crouch became an integral part of our North Port practice. His expertise and innovative mindset played a crucial role in the development and implementation of Epiphany Health Direct Primary Care, a pioneering model that has gained national recognition in the field.

Beyond his clinical practice, Dr. Crouch has made significant contributions to the community. He has served as the Medical Director for New College of Florida and has been a preceptor for the Yale University Physician Assistant program.

Today, Dr. Crouch continues to serve patients at our North Port location, where he provides comprehensive and compassionate care.

Dr. Hubman
Dr. Tiffany Hubman

Tiffany Hubman, DO| 

Dr. Tiffany Hubman, a compassionate and highly skilled physician, joined Epiphany Health in 2021 as the medical director of our DeSoto County location. Hailing from West Virginia, Dr. Hubman brings a diverse background and a strong commitment to serving others.

Dr. Hubman's dedication to helping others is evident from her service as a combat military police company member during Operation Iraqi Freedom and Operation Enduring Freedom in Iraq. Her time in the US Army reflects her unwavering commitment to protecting and assisting others in challenging circumstances.

Dr. Hubman began her medical journey at the esteemed West Virginia School of Osteopathic Medicine, where she obtained her medical degree. She pursued her passion for family medicine through her residency at Manatee Memorial Hospital, gaining valuable experience and refining her skills in providing comprehensive care. Her dedication and expertise have earned her certification from the American Osteopathic Board of Family Physicians.

Beyond her impressive qualifications, Dr. Hubman's genuine warmth and compassion make her an invaluable asset to the Epiphany Health care team. She strives to create a supportive and caring environment for her patients, ensuring they receive personalized attention and the highest quality of care.

Candice Nicoll, ARNP

Candice Nicoll, ARNP| 

 

Candice is a board certified Advanced Registered Nurse Practitioner and joined Epiphany Health in December of 2015.  Born and raised in Canada, she completed her undergraduate education at Douglas College in Vancouver, British Columbia.  Later she graduated from the University of South Florida with a Master of Science degree in Nursing. She is certified with the American Nurse Credentialing Center and the Florida Board of Nursing. Candice has a passion for patient-centered care. She has over a decade of primary care and women's wellness experience. She also has several years of experience in gastroenterology specialty care. Candice has expertise in the cure of hepatitis C with the latest advancements in treatment.  Candice serves patients in our North Port location.

 

Anne Horner, COO

Anne Horner| 

 

Anne is the Chief Operating Officer of Epiphany Health.  Originally from Vermont, she has lived in Florida most of her life.  She oversees the operations of all practice locations.  She is also responsible for community marketing and practice development.  She is the primary point of contact for new patients and business accounts. She has been with the practice since its initial founding in 2002, through its transition to DPC in 2010, and has served in nearly every capacity of the practice along the journey.
